Description:
Skin biopsy size of 1.4 x 0.6 x 0.5 cm showed on the surface a slightly elevated bluish formation with a diameter of 0.7 cm. The histologically received material is a skin fragment of hypercarotic and acanthotic epidermis with a symmetrical melanocyte formation built of spindle-shaped epithelial melanocytes located in the papillary and initial parts of the reticular dermis. The described melanocytes do not show a clear maturation towards the base by retaining prominent nucleoli until mitosis is present. Along the formation, medium-dense accumulations of lymphocytes are found. The formation showed a maximum thickness of 1,49 mm according to Breslow masurement. The described formation does not reach the tissue-marked edges of the excision, and is 1.5 mm from the nearest lateral edge while 3 mm from the base. In addition, nests of a preexistent dermal nevus are found.
The histologically and immunohistologically described lesion (HMB45, melan-a, Ki67, p16) shows atypical features therefore the finding in the received and examined material corresponds to MELTUM lesion of 1.49 mm thickness.

Anamnesis: Patient takes Euthyrox
Status: Postoperative findings without observation
Conclusion: Given the finding of PHD, a reexcision of the scar and biopsy of sentinel left lymph node is necessary. The patient is scheduled for hospitalisation on 16.08.2021.
The patient will be presented at the physician council for melanoma on 19.07.2021 and will be notified by phone on the physician council outcome.
